We bill many insurance plans for consults.

Self Pay
We are happy to see self-pay families as well -> $250 home visit and $175 office. After your consult, we will provide you with a superbill that can be submitted to your insurance company.
Families with the following plans will be self-pay:
- 🟦 All HMO plans
- 🟦 Healthshare plans
- 🟦 Tricare
- 🟦 Medicaid
- 🟦 BCBS State Employee Plan
- 🟦 BCBS FEP/Federal Employee Plan We are out of network, but with a great deal of persistence on your part *before seeing us*, reimbursement is possible: 65% if you have FEP Basic and up to 100% if you have FEP Standard. Please call us at 319-316-2004 to discuss.
Self pay rates: $250 home visit, $175 office. We will provide you with a superbill after your visit.
